I routinely run an attachment extractor that pulss attachments from 
messages and sticks them in a folder.  I then delete all the .dbx 
files with the messages the attachments were extracted from.  There 
are sometimes hundreds of messages.  They take forever to delete and 
then to empty from the deleted folder.  Deleting the dbx files is 
faster and OE has always recreated the missing folders without a 
hitch.
