Hi there Barrie,

I may be wrong, as I haven't thought this through greatly but...

Surely there is no need what-so-ever to document the accident, except in 
the case of apportioning blame, legal actions, subsequent investigation, 
etc.

The only real requirement following an accident is to make as best as 
possible sure that it doesn't happen again.

If documenting aids this, then yes, it should be done; but when so much of 
our equipment (and procedures) are custom made this is not a forgone 
conclusion.

In industry on the other hand, where many people use identical equipment 
and follow identical procedures, then there is almost always a 
need to document an accident, as there is likely a high risk others may 
suffer the same situation.

To do nothing and let the same accident happen again is indefensible. To 
write out a report and find next week someone had the same accident is 
also indefensible!!!

To do the appropriate thing (if this is broadcasting news of the accident) 
is the right thing to do.

In this case 'broadcasting' is almost certainly not writing it in a dusty 
accident book, where it may only be picked up upon by the club health and 
safety officer (if they exists) perhaps once a year.
