I believe some of this also happens because lack of understanding and training by the corporation producing it. I've found sometimes it's not an agency at all. I attended an information session recently at a hospital that was going to start producing their own registration, informed consent and other forms. They had purchased an embosser that came with a basic braille translation software. They were under the impression that everything would transcribe and appear just as the print by just a couple of simple commands. There was also a certified transcriber in attendance and we emphasized the importance of having the materials proofed by someone knowledgeable in braille before they were distributed to patients - particularly since some were legal documents! We were very impressed that the hospital was wanting to take the steps to make all materials accessible to patients in the formats they preferred, but definitely concerned about the misrepresentation or misunderstanding about how braille is produced. Rather than having a human transcribe your bill, it's put through a few macros that make it readable. I do find this preferable, since my life is my business and I don't need it mulled over by a lot of people at a transcribing agency. I'd rather just take the bad braille in this instance, just so long as it's readable.
