I would say such a book that is aimed at converting from one program to another, would benefit from such an index. Someone coming to MS Office from Open Office would highly benefit from such (E.G. "Save As: See Ribbon"), even though there should be a chapter dedicated to that, the results of a search of "Save As" is going to be a long one. Another thought: what about those that plan to print it out? Just because it's an eBook doesn't mean everyone is going to read it that way.
