It most cases only being able to open 8.3 names via double-clicking the file is due to Microsoft believing the application is n 16-bit program that only supports 8.3 names. To correct this when registering the file type, you should put double quotes around the %1 argument so it appears as "%1" in the file asssociation dialog.

Well, it's a new year, my support guy is back, and problem solved! It turns out that the *.pad files were not a known file type (RMB on the file to Properties, Type of File is "unknown"). Not sure why double clicking on 8 character or less *.pad files would load it into Pad Designer, but not over 8 characters, doesn't makes sense.
So setting it up in explorer (Tools -> Folder Options...) to open up all *.pad files with "pad_designer.exe" did the trick.
