[optimal] Re: No infiltration...no Fluorescein

I had it happen on a diabetic patient - what appeared to be a nice
antecubital vein, good push, no sign of infiltration, but absolutely no
fill.

Repeat injection on the opposite arm, perfect transit. The fellows thought
there was some sort of loop in the vein - a new vessel that basically went
nowhere. Really strange.
