Many many years ago, that very thing happened to me.  The doctor came in, used 
the 21G straight needle (ouch!) to inject the dye in the antecubital.  I 
started taking my pictures and everything was just fine.  45 seconds go by; 60 
seconds go by, 90 seconds and still no dye.  As I am franticly checking 
settings, power, filters he is asking why there is no dye.  Everything was set 
properly, patient had absolutely no signs of infiltration, no pain anywhere in 
their arm, shoulder or lower part of the arm.  The doctor had no idea why there 
would not be any dye, he said he got it in the vein.  He ended up re-injecting 
in another vein, and what do you know.  A perfect FA transit!  We chalked it up 
to the bizarre category.  I have never seen that since.

Has anyone ever experienced a phenomenon where an FA injection did not 
infiltrate, or at least we did not have any indication of infiltration, and no 
dye infuses into the retinal blood vessels? All camera functions we're verified 
and set correctly and the Color fundus and redfree images we're good.
Just curious.
