This book is a little old, and it's about software development which is not a 
1-1 match to web development, but when I was starting out I found it very 
helpful. It has the basic "best practices" for a staged handoff production 
model, with helpful tools and checklists. The title is slightly misleading. It 
doesn't really tell you how to survive the speed bumps you will encounter in 
every project, but it does give you the planning tools to hopefully avoid them 
in the first place. It's an easy read, as well.
