Martin,

If it's equivalent to a 27C64 then making copies and being able to modify it 
will be easy.  It should run with an EPROM in place of the OTP.  Is the OTP 
socketed so it could be removed and the contents dumped on a Programmer?  
Desoldering requires care though.

Ron, the chip is OTP without the window for 
erasing, so ties in nicely with your description for the two date codes.  
Yes I can post a photo, I will take one and upload a link.
