The more I think about this we actually need to get rid of the idea of
associating the semantic action files with file extensions and only use the
element name as a fallback. I think we can do this in BrailleBlaster. The
precedence needs to be:
1. Check the actual document type in the "<!doctype...> processing instruction.
2. Check the namespace for the root element and respect all namespace
declarations and prefixes throughout the document. (actually the latter should
always be done regardless).
3. If there is no doctype declaration or namespace declaration then base the
semantic rules to use on the name of the root element. This isn't reliable but
works most of the time.
The configuration files is a different matter but really, except for specifying
the semantic action file, it is often going to contain the same exact settings
for every XML file unless you really do want to change how the Braille is
formatted.

We need to implement this in BB even if it is not done in LibLouisUTDML because
it is the correct way to do it.
