It still seems to me like someone else suggested that when jaws is starting up it starts on another soundcard like a USB phone or headset or something like that especially as once you reload it it starts fine. Do you have any such usb devices? If so, temporarily unplug them and restart the computer, I bet jaws will start fine. Someone earlier also gave instructions to how to change the the default soundcard on which jaws starts.

JAWS can be a bit odd sometimes about which device it actually defaults to using and thereis an FAQ with the solution to this which I will add below as finding the thing on the website can be a bit of a pain.
