Hi Chrissuspect failed psu .. in which case it'll need returning to me to reflash via JTAG port. It'll also require a new PSU
Background: if the 3v3 rail is out of spec-high- the flash chip will refuse to program as a safety precaution. It will erase though!
One of the powersupply failure modes I have seen is an excessively high 3v3 rail
