Man, only one story? I'm sure most of us have more than one to tell..

I'd say my favorite has to be during an application upgrade - previous version 
of our application was not very PCI-compliant, stored credit cards, track 2 
data, you name it. The new version of the app was built on 10g rather than 9i, 
so to try to kill all birds with one stone, we came up with a process to 
export, wipe the server (DOD-strength-overwrite of the datafile directory), 
install 10g, import. Well, soon someone decided to script it. Everything looked 
fine during the migration / upgrade, but first reboot post-upgrade, when Oracle 
was down and nothing was holding the datafiles, the DOD-overwrite finally 
managed to wipe the location. Unfortunately, his script included the 10g 
datafile location, not the 9i datafile location....

Hey all -

I'm writing a paper about top causes of downtime.  As one component of 
research, I'd like to get some input from you!

One minute, two sentences. First sentence: describe what went down.
Second sentence: describe why.  (I have to categorize all of these.) Everyone 
should have at least one downtime story so I'm hoping for a lot of feedback!

Answer about any technology - database, operating system, etc.
