Hi Deborah,
When I Braille forms, I like to put 5 dashes to indicate a space to fill
in. But, as I think more about this, I realize that Braille readers are
not likely to fill out the Braille form since those requiring the form
will want it in print. The Braille form is created for informational
purposes so that the Braille reader can do one of the following:
1. Reproduce the form either on a PC or typewriter and fill it in.
2. Direct a reader in the filling out of the same form in print
3. Use a cassette or disc recorder to read the form along with the
answers.
Not sure if the Library of Congress rules speak to this issue.
