Looks like we both have things we can still learn:

I've been looking into this and whereas you can disable TalkBack from the
notification menu (hard to miss that one as it's right in front as you
pull it up lol), locking the phone does nothing on my S4. I found some bug
reports on the TalkBack bug tracker about this issue so it seems to be a
known problem.  However, an interesting comment I found when looking
through some developer stuff was that if you enable the Accessibility
Shortcut under Settings > My Device > Accessibility, it not only enables
the Accessibility option under the Power button menu, but also allows you
to hold in the power button and hold with two fingers  which will start
TalkBack from a disabled state, just as one did when starting to use the
phone for the first time.  Nice that, and not something mentioned within
the Settings app so perhaps not an addition that's widely known.
