Hi Padma.OK, when the document opens in Adobe Reader, it is presented in a format similar to a web page. The only difference between read only documents and forms is that forms have editable fields and other controls which you can interact with. You cant accidentally modify the main document, only the contents of the fields in which the author has allowed you to edit. With the ESA 50 questionnaire for instance, there are fields to allow you to describe your condition and radio buttons and tick boxes as well as the usual elements you'd find on a web page including headers so you can navigate the various sections of the form.
