High and low on a bare minimum of all four corners.
One possibility is that the organizational mindset had become "this is
now routinely operational and no further surprises will occur" - thus
even relatively cheap measures to handle surprises are dispensed with -
not just to save the modest costs involved, but as an expression of
organizational confidence.
It's a logical error, given the huge cost of even low-likelihood
surprises and the (relatively) small cost of a few extra cameras,
survivable data collection, etc. But the error is a common one.
