Is it a case where you need the dev versions installed?

There's no sdl-devel* package listed in the requirements for the FreeBSD dosbox package:

$ pkg_info -r dosbox-0.65_1
...
Dependency: sdl-1.2.11,2
Dependency: sdl_net-1.2.6

$ cat /var/db/pkg/sdl-1.2.11,2/+DESC
This library is designed to make it easy to write games that run on UNIX,
Win32 and BeOS using the various native high-performance media interfaces,
(for video, audio, etc) and presenting a single source-code level API to
your application. This is a fairly low level API, but using this, completely portable applications can be written with a great deal of flexibility.

$ cat /var/db/pkg/sdl_net-1.2.6/+DESC
This is a small sample cross-platform networking library, with a sample chat client and server application. The chat client uses the GUIlib GUI framework library.

I do haver libsdl1.2debian-all installed. As I don't know what exactly "set the SDL_CONFIG environment variable" means - I have a true problem with environment variables- and how to set its value, I gave up.

  If (as is almost certain) you're using bash,
$ export SDL_CONFIG=<path-to-sdl-config>

in FreeBSD, that would be /usr/local/bin/sdl-config
in Slackware, /usr/bin/sdl-config
If you can't find the full path in Debian, do
$ locate sdl-config

  Also works in Slackware, but not tested elsewhere,
$ export SDL_CONFIG=`locate sdl-config`
