I don't have to do all that though it may help if you are having a problem. I
don't have to go anywhere near my books and I don't have to check whether or
not they are downloaded at least not on my fire 8 HD; haven't tried it on my 5.
The book doesn't have to be downloaded as far as I can see unless of course you
are not connected to the cloud. I just tried two different books that were
definitely not downloaded and they began to read and still didn't download; I
suppose they might be slower or could stop more easily if not downloaded. Also,
I don't have an Echo either so that isn't the problem. I sure hope you can
find the cause; I know it has to be really frustrating!

want.
as for alexa, after the chime simply say read "your title here".
