If you want to use through hole parts, just drill a hole through the pads. The LM1117 is a TO-252 package, east to get and easy to solder in place, easier than the si570. Perhaps the voltage output lead is not being used. Second, is there a compelling reason for using surface-mount devices for the 22 pF capacitors attached to the 16 MHz crystal? I can understand that leaded parts would introduce inductance, but I wouldn't think that the clock speed on the Arduino would be all that critical. There is certainly enough board space to use through-hole capacitors where they are located. It would be nice to minimize the number of components with specialized packaging in order to reduce cost and increase the likelihood of part availability.
