Just to clarify, do you only want the use item skill to make a sound
when items don't already play a sound when used?

No, I meant if you have sound= in the file of the item you want to
use, then it'll make that sound when you use that item.
And the skill fail thing works for me too.

Success and failure sounds for skills can be done through the engine.
If you want a skill to fail, name the skill fail sound the name of
the skill with fail after it. Like blaze_fail. Alternatively you can
have a generic skill fail sound called skill_fail that will play
instead.
Items already play a sound when used, if you specify that in the
file...
