First you need an account for dev.haiku-os.org, which sadly is separate from
that
of www.haiku-os.org. The account needs your email needs to be verified before
bugs can be posted. (This has been a stumbling stone for many.)

After that it's not so hard. It's basically just a form in which you describe
the bug.

Summary: "Haiku don't run on my real hardware"

Description:
"Haiku don't run on my real hardware.
It's a Brand X, Model Y and it crashes at bootup. (KDL output included)
The audio/media server or driver appears to be involved in the crash."

Select Type: bug (since it's not an enhancement)

Select "R1/development" if you're testing a nightly Haiku image rather than the
latest alpha.

Select Component: system/kernel (since it KDLs) or servers/media
or driver/audio/hda, if you suspect that is the culprit. Try your best but
know that someone will come along and correct it if it's wrong, so it
doesn't matter all that much.

Skip all other fields.

(Especially the keywords. Pet peeve of mine.
There's hardly any value in keywords in the face of context search.)

There's no need to CC yourself since you will be in the feedback loop of any
follow-up comment. As will anyone else who follow up with a comment, I think.

If you have the means to get a syslog, please upload that, and a text or photo
of the KDL backtrace. Unzipped. Separate files.
