Do you mean that we should leave some blank paragraphs after each heading?

Unfortunately, the document isn't mine and the problem is worse when we
cross reference to bookmarks because:
- some of the authors don't realise that there is a bookmark and include the
heading in that, or
- the bookmark is not put over any text, so there is no text to pull into
the launch site. (This method of inserting bookmarks is the author's
favourite and she's a much more experienced tech writer than me.)

Janet - this is not an indication that the file is corrupt. This behaviour
occurs if you edit a heading then press return and start entering text,
which of course it is allmost impossible to avoid doing. Try and get in the
habit of putting a few blank returns in after all headings, but it is
especially important to ensure they exist before including a heading in a
cross-ref.
