This happened with one of our users, too, and the only thing I could find was some kerberos errors related to the user's workstation.. ?? If the accounts are locked out, someone is trying those user accounts and entering the incorrect password more then 3 times. We have a 3 failed attempt policy that will lock users out but some of these users are not even failing once.
