For what it is worth, there is an LM321AH op amp. Part of the series LM121AH, LM221AH, LM321AH. Also available in a 14 pin dip package as LM321AD. But if the supply regulates and adjusts at all I would suspect the IC is OK. Maybe some of the old resistors have changed in value.
