Good morning,
We use a product called Wufoo (subsidiary of survey monkey) for simple forms
and routings. It’s inexpensive, can be used internal and publicly, and power
users can build their own forms or have IT help.
The form processing can be done through Wufoo administration tools, and/or
we’ve found a way to have incoming results populate into SharePoint lists for
more robust processing.
While not a full blow enterprise form/routing solutions, it’s been a great
“Keep It Simple” tool that our users really like. Great way to get started.

We also use Laserfiche Forms. The product is well designed and the integration
with Laserfiche document management is a big plus.
Our use of Laserfiche Forms is primarily for public facing forms - anonymous
fillers. Unfortunately, as already mentioned, the licensing model is
(ridiculously) cost prohibitive for any kind of widely used internal form
requiring workflow and authenticated approval. If this is your target feature,
don’t waste your time and money. Look for something else.
