For longer lenses consider also the Novoflex follow-focus lenses with the squeeze grip. The best of them is the 400mm f/5.6 T-Noflexar with either the PIGRIFF-C focus grip or the PIGRIFF-B grip along with the BAL-U extension bellows. Mine was US$125 plus Leica-R adapter (Adorama or B&H). My informal tests suggest that the T-Noflexar is optically nearly as good as the 400mm f/6.8, at the cost of more weight and bulk.
