While I still haven't seen the dialog, and still think it's a good idea, I also like Ingo's suggestion of having some kind of context menu that offers the obvious choices. However, Mac OS X has enough flaws itself, and while it's supposed to be beautiful and usable, it's not really something I would usually copy from without questioning it a good deal first.

When I first opened the current Keymaps preflet it was not
particularly intuitive and I still don't think I can use it all that
well.

Which is certainly not being solved by adding a dialog, though ;-)
Besides the unclear d&d behavior, I think the main reason it's unintuitive is because it offers functionality not found in other systems -- you just don't expect it.
