It's probably more like end result.  For scrollers the designer of the
scrolle will want to predictably place certain items on the ground as ground
targets.  Random generation will need to be augmented to include pre seeded
ground elements.  A little wrinkle to the randomisation issue.  

Also players may subconsciously pick up on background elements to know when
to prepare for bigger enemies and position appropriately.
