This may be a bit of a longshot, but I've had some issues in the past with
McAfee blocking USB ports.  I'm not on-site right now, but take a look at
the DLP policies if you are using ePO.

Tony,

These don't have extenders or hubs, direct to the USB ports in the back of
the WP.  It happened using both the regular and high-speed ports.  One of
them has a USB isolator and it also experienced the problem.  In the past it
was rare to have to push/pull one, it happens sometimes on stations with a
Remote Graphics Unit.

Our control rooms use the trackballs and we have not had issues with them.
On a rare occasion we will have to push/pull the USB and it works again, but
this is not very often. We do tend to buy new ones with computer upgrades
every 3-4 years. Is it directly to the machine or is through the USB
extender with the hub? We have more often had to either push/pull the
keyboard/mouse/trackball or pull power from the hub and re-power.

Yesterday during a startup from an outage, we had an experience where an
operator was operating from 3 different Workstations (Win 7, I/A 9.1, FV
10.4).  Several times on each WP the Foxboro approved trackball would stop
moving the mouse pointer.  Push/pull of the USB plug would get it started
again.  There was very high usage of the WP's at the time.  Just wondering
if anyone else has seen something like this?
