A general rule is: rubbish in, rubbish out
It is good to simplify the Word Document as much as possible before
putting to Duxbury.
Some general tips I use to speed this up:
1. Highlight whole document and set to whatever your default font and size
is that you generally use.
2. Highlight whole document and remove all paragraph formatting (ctrl-q)
This will set all paragraphs to normal.
3. Scanning will often put in bold and italics where not needed. If you
are retaining these typeforms, then you need to check your document
accordingly. If you are not needing them in braille, then highlight whole
document and remove all character formatting: ctrl-spacebar
4. Add headings where you want them: ctrl-alt-1 sets a paragraph to a 1st
heading level, ctrl-alt-2 to a 2nd heading level.
5. Duxbury will now import your file with basic formatting, normal
paragraphs and headings. You can now add any additional formatting you
wish.

There is more you can do, but as I am in Australia, some of our formatting
rules are different and we use Unified English Braille so I am reluctant
to go further than this.
