How do you host your wiki? I had a strange (to my mind) issue: when I set 777 (666) php applications reported permission problems. My web hosting provider said I should set 755 (644 for files). And it indeed helped, although being more restrictive. Everything worked fine with these settings. So I suggest you ask help from your provider, if you do not host the wiki yourself.
