You have it right, but there still needs to be the name of the copyright
holder along with a date.
There are a number of books sitting in the Step 1 pool which have been
returned for vague copyright info.
One book, namely "Boot Camp for Christians,"
has been sitting there seemingly forever.
I have attempted to research this one and draw blanks.
The warning is "don't specificly scan something for upload unless you
first can find the copyright info."
Otherwise, for legal reasons, the book ends up in limbo regardless of an
excellent scan quality and validating.
