After examining some of the results of a Google search, I believe that this Malicious Software Removal Tool after installation runs once in the background, attempts to remove certain worms etc., and then deletes itself. Also it appears that Microsoft will be issuing new versions every Tuesday. People who use automatic update will presumably have their machines "cleaned" weekly, those of us who do not will have to remember to use the Windows Update weekly. That is how I understand it, but please correct me if I am wrong.
