[TCUG] Green Conflict

Hi,
 

I have a shuttle working signalised bridge where following a head on
crash both drivers claimed to have green at the same time. 

 

We tested the green conflict afterwards and as expected the controller
shut down the lights. 

 

Has anyone ever experienced fault conditions where green conflicts occur
and where the controller has also failed to shut down ?
