I have done all my scans, everything seems fine.  The slowdown is mainly
when the pc is rebooted or cold started.  Maybe it is other applications
running at restart or reboot causing my slowdown.  

I have deleted a bunch of the larger emails and turned off Journal.

If you don't use the journal I would try turning it off and see what
happens.
Any possibility you might have a virus / malware infestation?
Is the slowdown only apparent in Outlook?

noticed that Journal is turned on, does that slow Outlook down?
 
This is running on WinXP Home SP2
