Gene,

very sorry about the diagnosis. I've got some life-changing medical issues, 
including heart failure, so my sympathy is quite earnest. Best of luck with 
your treatment and keeping your spirits  up, and of course good luck dealing 
with the privacy problem arising from this technical inconvenience. 
ironically, something  that may seem like a wonderful advance in 
communication and medical record keeping may not be such a "bag of chips" to 
someone who can't see to read and write or use some gadget.

I have not had experience with this type of survey tool.  But, depending on
the types of questions, would it be possible to record the answers in some
other format (MS Excel perhaps) and then to have a para-medical professional

enter your responses into the system when they are able?
