Cancer Survey

I have been diagnosed with and receiving treatment for esophageal cancer.
There is a survey that the treatment center would like me to complete, at
least, once a week. The survey appears on a device that seems similar to a
laptop. However, in order to answer the questions, there is a touch screen
and, of course, the thing doesn't have a screen reader on it.

I don't particularly sharing my answers to the medical questions while a
sighted individual marks them for me. But, there are several psychological
questions that I am not answering because I don't feel comfortable sharing
those thoughts and feelings with anyone: except a qualified para-medical
professional.

Has anyone who has had a similar experience, cancer notwithstanding, found a
means by which to complete surveys of this type without sighted assistance?

Thanks in advance.
