I think the problem, in the situation Tiff's brings up, is that Gustavo
just takes the word of the submitter/validator that the newly submitted
replacement is better.
No comparison is done; and so much is subjective anyway.
K1000 scores, of course, can be one comparison.  But then someone else may
suggest that the formatting is better, the book is less prone to stripper
ruin, the page breaks are more true, whatever.
Without standards in this area, this just becomes a can of worms.  And
without sufficient time for an administrator to examine the situation,
Bookshare on its end can hardly be expected to make a decision.
I understand Tiff's frustration, but given the nature of the BookShare
system and its resources, I doubt that this can be dealt with.
