Michael:

Given that its origin seems to suggest an original meaning of "going out" 
,  while other meanings seem to break away to refer to "outstanding" 
matters, can we assume the legal profession is to blame again.?

Interestingly, I've just come across "issue" in something I wrote
myself quite recently: "What issue (problem or opportunity) are you
dealing with?" There I used the word to mean "something that needs
attention". So yes, it includes problems, but it includes other things
besides.
