I found something I wrote about five years ago in which I suggested to students that they should avoid using the word 'problem' in reports "because mamagers often don't like to be told they have problems". Managers are employed to ensure that problems don't arise, so to suggest that they have problems is to suggest that they are not doing their jobs. I wouldn't recommend criticising a manager if you're a junior employee.

Like others, I had a look at the "difference between" site and like others, I disagreed with it. Would I like to replace it? No thanks, but if I had to take a position, I'd say that an issue, left untreated, could worsen and develop into an incident or a problem.
