That default is gone now. The default is to always speak passwords. I know
this because I have done several resets. This is true on Android 10 or 11.

Regarding your first question: In TalkBack you can decide to hear password when
typing. It's default value is set to speak when you have headphones on, but tha
t can be changed, so that you hear the keypad also whan you don't use
headphones.
