The problem prevents the applist page from displaying, if I remove the
user rights to the specific app having the trouble the page is fine...
But I currently cannot change anything as it will cause a new icon to be
created and the problem comes back again

Permissions look quite normal (the first thing I checked)

Since posting however I have found that WebDAV has been disabled on this
particular IIS server, but I'm not sure whether Nfuse needs WebDAV
enabled when communicating.... Anyone know???... Doco seems scarce on
this topic

>From what I am reading, it looks like it is a problem with the icon in
NFuse.  If that is the case, verify that the IUSR_computername account
has 
full permissions on the NFuseIcons directory.  That is the account 
responsible for creation of new NFuse icons.

Does it prevent the app from launching or just have a dead icon?
