Hi Pat -- regarding transmission line references, I would suggest the one that I always suggest on this list, "Analysis of Multiconductor Transmission Lines" by Clayton R. Paul. Among other things, it has a 100+ page chapter devoted to the per-unit-length parameters, including the physical meanings of R,L,G,C, both diagonal and off-diagonal elements. The above reference also gives the definition of the capacitance matrix, which if I'm understanding your usage, is the same as what you are calling the electrostatic induction matrix. A matrix whose diagonals contain only the capacitance of the conductor to the reference conductor is *a* capacitance matrix, but it is not *the* capacitance matrix. In lieu of launching into an explanation, I would point you to the text given above, or probably any other transmission line theory text. They're certainly more authoritative than the various user manuals. Or, if memory serves, Eric Bogatin wrote up a short piece a couple of years ago dedicated to the capacitance matrix and common misconceptions surrounding it.
