You are correct. A weakened immune system usually triggers the mange.....so
the question really should be what is causing the dogs weakened immune system.
Unless that is hereditary, I'm not sure why you would spay/nueter the dog.

As I understand, demodex lives on every dog, and times of stress may bring
it out - growing-up is stressful, so you may see it here & there on a puppy.
I am sure I will be corrected if I am wrong.
