It seems the form that is currently on the SAC website will do best. The
prices match the one in the "4th quarter" issue of the newsletter but the one
on the website now correctly indicates David Fredrickson as the SAC Treasurer,
and his address for sending the check. So I'll use that one.

Use the form in the December issue of SACnews ad it reflects the dues increase
that went into effect Jan 1st.
