What you suggest appears to be correct.
Entertainers are often known by a different name, along with those who for
one reason or another choose to be known by something other than their
correct name.

FISHER, Grace Mercy and then in other name Van BEERS, Helen. The name in
brackets doesn't sound like a nickname,
or do I put it in brackets like the notice.
