I have used Fake Name Generator (http://www.fakenamegenerator.com/order.php) several times. The free service lets you select the country, language, fields, though it is limited to 50,000 for a single order (though you can have 3 outstanding orders at any one time). If you order it in sql format, it also provides the table ddl.
