I am sorry to hear that you are having trouble..
This issue is because in Heyex 1, you need to ensure only one person is in a
chart at a time. This can cause corruption when there're too many cooks in the
kitchen. I would make sure that the physicians understand that they should not
be accessing a chart before you have returned the patient to them.

My Spectralis keeps losing data
I always open the file, check before patient leaves
The pattern seems to be our busiest Retina day
Fellow gets ahead of me, opens file while I am working on patient
Could this be the loss of data, or a server hiccup?
