A compounding issue can be people opening up a record and exiting the app
before properly releasing said record. This is especially true if it happens on
a "capture" station as opposed to a purely review station.

I am sorry to hear that you are having trouble..
This issue is because in Heyex 1, you need to ensure only one person is in a
chart at a time. This can cause corruption when there're too many cooks in the
kitchen. I would make sure that the physicians understand that they should not
be accessing a chart before you have returned the patient to them.

Fellow gets ahead of me, opens file while I am working on patient
